It also requires clear, unambiguous collaboration between the various professionals – from the specialist prescribing the treatment and the pharmacist supplying and monitoring the medication, to the transfer nurse accompanying the patient, and the nurse who carries out the treatment once the patient is home. As soon as the doctor, nurse, pharmacy or outpatient pharmacy logs on to Yulp with a request for treatment, Yulp immediately (in real time) contacts a locally available nurse who is trained for that specific treatment. Through Yulp, she/he is then informed about the patient’s medical background and the treatment prescribed.
